Request for Special Meals and/or Accommodations – must be completed by parent/guardian and signed by a Medical Authority. This form must be filled out annually and kept on file in your child’s school health office and the Food Service Department.

The Food Service Department is always very active in preparing for the new school year. We realize nutritional healthy meals play an important role in the education of our students. Studies have proven children learn better with a nutritious meal in their system than those that are less fortunate. So with this in mind, we are anxious to introduce the Healthy Hungry Free Kids Act (HHFKA) passed as a law by congress. To file a program complaint of discrimination, complete the USDA Program Discrimination Complaint Form, (AD-3027) found online at: http://www.ascr.usda.gov/complaint_filing_cust.html, and at any USDA office, or write a letter addressed to USDA and provide in the letter all of the information requested in the form.
